Every weekday, a multidisciplinary team, including Medicine, Nutrition, Occupational and Physical Therapy, Psychiatry, Social Work and Speech Language Pathology, meets to review patient cases and form team goals. This unique communication effort is designed to safeguard the quality of patientcare by uniting the care team around resolving patient needs andimplementing a collaborative treatment plan. Results include improved patient safety, outcomes and speed of discharge. |
