With a focus on service excellence, nurses coordinated an innovative approach to patient care that includes monthly meetings with all the caregivers on eachcase to identify solutions to patient concerns and to develop individualized solutions to address each patient’s unique needs. Through weekly community meetings, as well as focus groups, patients are able to give the team valuable feedback. The high rate of return of patient surveys and the faster resolution of issues have resulted in greater patient support and satisfaction. |
